ACCESSING NDIS SUPPORT SERVICES
Three Little Steps, One Big Help - That's Our Promise!
1. Getting to know you
We know that it’s really important you feel comfortable and connected with your potential carer/support worker. That’s why before any paperwork or formalities, we like to jump on a call to get to know you!
During this brief chat we’ll focus on getting to know each other and understanding what matters most to you. This is also a good chance for you to ask us questions to get a sense of who we are, and how we operate!
2. Getting the ball rolling
After our intital chat, if you are still happy to continue we will get the ball rolling by asking you to complete our participant intake form.
The form has a series of questions, aimed at providing us all the information we need to provide you the best quality care, tailored to your unique needs.
If you have any questions or concerns, or simply need a little bit of assistance to complete the form we will still be there to help!
3. Making it official
In the final step, we will take the information you provided during step 2 and write up a service agreement document.
The service agreement will cover specific details about the supports you will receive from us, how items are charged within the NDIS and all the terms and conditions.
To make things official, the service agreement will need to be signed by both parties as a formal agreement of what we will be delivering to help you reach your goals. It’s what we like to call a paper handshake!
Once the documents are signed we’ll arrange a 30 minute meet and greet to match you with the best support worker.
Testimonials
We're proudly parent-approved!
Jasmine was such an amazing person and a blessing to our Family. We have been away from Australia for a longtime and the process for NDIS has changed so much over the years. I am lost with some much of paperwork to restart the process for my daughter. It was a nightmare. Luckily we found Jasmine who is an amazing person and best mum. Having a special needs kid herself she knows that this path isn’t easy. She participated with me in my daughter’s NDIS review. Assisted with my paperwork and made the process seamless. Without her it would be very difficult for us to get our daughter’s funding restarted. She has immense knowledge and loads of information.
You’re amazing Jasmine, you really do understand the complexities we face
We absolutely love Judit and really appreciate how she wants to work on our goals. She learns very quickly and we’re so grateful for her support. She is very passionate for what she does and it shows!
